Job summary

Oxford Health NHS Foundation Trust in Buckinghamshire is seeking a Band 5 Occupational Therapist to join a brand-new Intensive Case Management and Rehabilitation Pathway. You'll work predominantly in community settings with adults and older adults to build independence and recovery.

You will receive strong supervision, a structured development plan and opportunities to shape an innovative service while managing your own caseload and driving delivery in people’s homes.

Qualifications

• Full valid driving licence and access to a vehicle.

Responsibilities

• Deliver high-quality Occupational Therapy assessments and interventions for adults with complex mental health needs.
• Help people develop skills, confidence and independence to live fulfilling lives in their communities.
• Provide assertive outreach by meeting people where they feel most comfortable, including homes and community settings.
• Build therapeutic relationships with individuals who may find traditional services hard to engage with.
• Work with colleagues across health, social care and voluntary services to provide joined-up care.
• Contribute to recovery planning, risk management and discharge planning.
• Maintain accurate clinical records and participate in service development, quality improvement and audits.
