Job summary

HCRG Care Group is hiring a Band 5 Community Occupational Therapist to join a multidisciplinary team. You will assess, plan and implement occupational therapy interventions for patients in community and outpatient settings, aiming to maximise independence and wellbeing.

You will work with senior clinicians, contribute to care planning, documentation and professional development, and provide patient-centred care in line with NHS terms and conditions.

Qualifications

• Degree or Diploma in Occupational Therapy
• HCPC registration as an Occupational Therapist
• Full UK driving licence and access to transport suitable for community working
• Evidence of continuing professional development
• Experience of assessment, care planning and delivering occupational therapy interventions
• Ability to work effectively within a multidisciplinary team
• Strong communication and interpersonal skills
• Ability to work independently, manage a caseload and prioritise workload effectively
• IT literacy and accurate clinical record keeping skills
• Flexible, adaptable and committed to delivering high-quality patient-centred care
• Teaching and assessing experience in clinical practice
• Further qualifications relevant to occupational therapy
• Awareness of recent government policy affecting future clinical practice
• Evidence of an innovative and flexible approach to care delivery
• Community-based clinical experience

Responsibilities

• Delivering Occupational Therapy assessment and treatment for patients in community and outpatient settings
• Undertake comprehensive assessments, develop and evaluate care plans, and document reviews
• Work with a multi-professional team to coordinate care and outcomes
• Participate in supervision, appraisal and ongoing professional development
• Maintain professional standards, infection control and information governance
