• Registered Nurse in Learning Disability or Mental Health nursing with current registration.

Responsibilities

• Clinical responsibilities include health needs reviews and holistic assessments.
• Identify and manage risks to physical and mental health.
• Plan and coordinate future care in partnership with patient and carers.
• Liaise with Primary and Secondary care and other agencies.
