Job summary

Devon Partnership NHS Trust is seeking a full-time Learning Disability Liaison Nurse to provide specialist support across acute care, CHC and services for adults with learning disabilities who are at risk of crisis or hospital admission.

You will hold a defined caseload as lead practitioner within the SHCLDT, collaborating with families, carers and health and social care partners to deliver joined-up, person-centred care.

Qualifications

• Registered nurse with learning disability specialty (RNLD) or RMN/RGN with relevant experience.
• Evidence of ongoing professional development relevant to learning disabilities.
• Willingness to travel independently across Devon for community visits.

Responsibilities

• Hold full clinical and professional responsibility for assessment, planning, implementation and evaluation of care for a defined caseload.
• Ensure care plans are person-centred, outcome-focused, and reviewed regularly.
• Support health checks, action plan reviews and transition processes.
• Implement POSITIVE behaviour support strategies and collaborate with providers and families.
• Contribute to safeguarding workflows and DSAP procedures as required.
