Job summary

Kent Community Health NHS Foundation Trust in the West Kent area is recruiting a Learning Disability nurse to join our community nursing service within the Learning Disability Team. The multi-disciplinary teams work across four localities and partner with social care to provide a coordinated service for people with learning disabilities.

Qualifications

• Level 6 Nursing qualification with registration in the UK is required.
• Recent healthcare experience, ideally in learning disabilities or mental health.
• Registered with the Nursing and Midwifery Council.

Responsibilities

• Assess, plan, implement and evaluate holistic care linked to prescribed care packages.
• Develop integrated care programmes with the client.
• Supervise and develop junior staff and students.
• Work in a multi-professional team to deliver high-quality care.
• Carry out clinical interventions to support individual and group therapy outcomes.
• Manage a caseload of adults with learning disabilities with complex needs.
