Job summary

You will work across nursing functions including Continuing Health Care and Primary Care Liaison, performing assessments, formulating care plans and delivering person-centred interventions.

The post is Band 5 under Agenda for Change, a 12-month fixed-term contract, and full-time with flexible working options.

Qualifications

• Excellent communication skills and ability to engage with people with learning disabilities and carers.
• Knowledge of Learning Disability Services and relevant UK standards.
• Understanding of Mental Health Act/MH Capacity Act and CHC framework.

Responsibilities

• Work across nursing functions including Continuing Health Care, Intensive Assessment and Treatment, and Primary Care Liaison.
• Undertake assessments and formulations, develop and evaluate care plans and interventions.
• Provide person-centred care and work with families, carers, and other stakeholders.

Skills

Education

RNLD or Equivalent RN Qualification
