Job summary

HCRG Care Group is seeking a Band 6 Community Nurse / District Nurse to deliver high-quality, person-centred nursing care within the community from Beacon House, Devizes. You will conduct comprehensive assessments, plan and coordinate care, and deliver evidence-based interventions with a multidisciplinary team.

You will work autonomously, using your clinical knowledge and experience to make informed decisions and manage complex situations.

Qualifications

• NMC/1st level registered nurse status.
• Full UK driving licence with car access.
• Proficient in Microsoft Office (Outlook, Word, Excel).

Responsibilities

• Understand and act in line with NMC professional standards.
• Work in a mixed-skill team, organise, prioritise and plan workload.
• Use assessment tools to inform patient needs and risk.
• Identify abuse/self-harm risks and safeguarding issues.
• Provide health promotion advice to patients and carers.
• Provide clinical supervision and support to less experienced staff.
