Job summary

HCRG Care Group is recruiting a Community Nurse (Band 5) to join our Melksham Community Hospital team in Wiltshire. You will deliver high-quality, person-centered nursing care in patients’ homes and within community services, aligned with AfC terms and NHS Pension.

Qualifications

• NMC/1st level Registered Nurse qualification.
• Strong communication and organisational skills.
• Full UK driving licence with access to a car for work purposes.
• Confident using IT systems and Microsoft Office (Outlook/Word/Excel).

Responsibilities

• Assess, plan, deliver and evaluate nursing care in partnership with patients, carers and the multidisciplinary team.
• Monitor patient progress and escalate changes as needed.
• Prioritise and manage a caseload in line with patient need.
• Maintain accurate patient records using electronic and paper-based systems.
• Deliver evidence-based care and contribute to quality improvement initiatives.
• Promote health, wellbeing and independence for patients and carers.
• Participate in team meetings, handovers and mandatory training.
• Support learning and development of students and colleagues.
• Ensure safe environment through risk assessment and infection control standards.
• Manage and maintain clinical equipment and stock.
