Five Rivers Health and Wellbeing Centre is seeking a Community Nurse Band 5 to provide high-quality nursing care to patients in their own homes. You will work within a multidisciplinary team, assessing needs, planning and delivering care, and promoting independence while adhering to the NMC Code.

You will be an NMC registered nurse with strong communication and organisational skills, capable of autonomous work and contributing to service development within the community team.

Formación

• NMC registration as a nurse (1st level) and current practising status.
• Strong communication and organisational skills are essential.
• Ability to work autonomously and within a multidisciplinary team.

Responsabilidades

• Deliver high-quality, person-centred nursing care in patients’ homes.
• Assess needs and plan care in line with policies and standards.
• Maintain accurate patient records and support clinical audits.
