Job summary

Leaders in Care is seeking a Community Nurse to provide hands-on nursing care in patients' homes across Milton Keynes and the wider Buckinghamshire area. The role offers autonomy, a Monday-Friday daytime pattern, and no nights or regular weekends.

Salary up to £39,000 DOE with a choice of a company car or a £4,877.50 car allowance, plus mileage support. You will work under strong clinical leadership, manage your caseload independently, and focus on high-quality patient care while developing

Qualifications

• Active NMC PIN required.
• Adult nursing experience in hospital or community settings preferred.
• Strong clinical assessment and communication skills are essential.

Responsibilities

• Conduct holistic clinical assessments and develop care plans.
• Deliver safe, evidence-based nursing care and treatments.
• Monitor patient conditions and respond to changes.
• Educate patients, families and carers; liaise with GPs and multidisciplinary teams.
• Maintain accurate clinical records and uphold governance standards.

I'm recruiting for a Community Nurse to join a leading specialist healthcare provider, supporting adults in their own homes across Milton Keynes and wider Buckinghamshire.

This is a great opportunity for an experienced RGN looking to move away from the pressures of a busy ward while continuing to deliver high-quality, hands‑on nursing care.

The Role

Working independently in the community, you'll manage your own caseload while receiving support from an experienced multidisciplinary and clinical leadership team.

Your responsibilities will include:

• Completing holistic clinical assessments
• Developing and reviewing personalised care plans
• Delivering safe, evidence-based nursing care and treatment
• Monitoring patients and responding to changes in their condition
• Supporting and educating patients, families and carers
• Liaising with GPs and wider multidisciplinary teams
• Maintaining accurate clinical records and high standards of clinical governance

About You

• Registered Nurse (RGN) with an active NMC PIN
• Adult nursing experience within a hospital or community setting
• Strong clinical assessment and communication skills
• Confidence working independently and managing your own caseload
• Full UK driving licence
