What is the job role?

This is an exciting opportunity to work with individuals experiencing homelessness, mental health crises, and those at risk of psychiatric inpatient admission. You will join a busy Home Treatment Team promoting the bio-psycho-social model of mental disorder, emphasizing the social model of practice. Your work will be stimulating and challenging, collaborating closely with NHS and Local Authority Homelessness Services, including commissioned and non-homelessness accommodation providers, as well as an experienced team of Social Workers, AMHPs, Psychiatrists, Nurses, Support Workers, and partner agencies.

You will be expected to:

  • Support individuals requiring urgent social support, focusing on preventing homelessness and sustaining accommodation.
  • Complete personalized assessments of social needs.
  • Assess under the Care Act (2014) and arrange social interventions aimed at recovery and wellness.
  • Collaborate with statutory and third-sector colleagues to assess, identify, and implement social and recovery interventions.
  • Always respond with compassion and respect, inspiring hope and recovery.

This post is exempted under the Rehabilitation of Offenders Act 1974; appointment is conditional upon a satisfactory DBS check.

  • Authentic, committed, and respectful practice at all times.
  • Excellent teamwork skills.
  • Knowledge of social issues impacting individuals with mental disorders and available interventions.
  • Understanding of homelessness and its challenges.
  • Ability to assess risk and escalate appropriately.
  • Creative problem-solving skills, including use of direct payments.
  • Ability to mobilize relevant agencies and resources and build strong relationships with individuals, families, carers, and partner agencies.
