Job summary

Catalyst Support is seeking a Recovery and Connect Worker to provide intensive, person-centred support to adults with mental health needs, helping them access community services and opportunities. You will travel across the service area and work with clients, families, carers and clinical teams, coordinating holistic support with partners, and attending multi-disciplinary meetings.

Qualifications

• A relevant diploma or degree in counselling, psychology, social work, probation, mental health or Level 3 Health and Social Care.
• Two years' community experience supporting adults with mental health, wellbeing or behaviour change.
• Strong knowledge of mental health issues, safeguarding, confidentiality, consent, information sharing and GDPR.
• Experience of collaborative working within multidisciplinary statutory and voluntary-sector teams.
• Excellent engagement, communication and written skills with professional boundaries and accurate records.
• Ability to travel across Surrey and work flexibly across the service area.

Responsibilities

• Manage a caseload and build trusted relationships with clients, families and carers.
• Develop and regularly review holistic support plans using motivational interviewing and behaviour-change approaches.
• Lead as the professional link with clinical teams, care coordinators, GPs and voluntary partners to connect clients with services.
• Arrange and attend multidisciplinary meetings and supervision, refer onward and support safe transfers.
• Maintain accurate case records, risk assessments and data; contribute to monitoring and quarterly reporting.
• Travel across the service area to provide community-based support, including occasional cover in North East Hampshire.
