Job summary

Southern Health NHS Foundation Trust is recruiting Mental Health Support Workers to join a newly established Community Rehabilitation Team (CRT). The role focuses on helping people with complex mental health needs to live independently, develop recovery and maintain stability within the community.

You will work with a multidisciplinary team to deliver person-centred support and build skills, confidence and social networks.

Qualifications

• Full UK driving licence and access to a vehicle for work use are required.

Responsibilities

• Work directly with service users to support recovery and wellbeing.
• Deliver structured interventions and activities as part of care plans.
• Support individuals to manage their social and practical needs.
• Assist practitioners in assessment, care planning and review processes.
• Build supportive relationships to encourage engagement and independence.
• Contribute to a safe, responsive and recovery-focused service.
