Job summary

Catalystsupport is looking for a Community Connector – Mental Health Professional in Guildford. This role will support the development of integrated primary care mental health services. You'll work closely with a multidisciplinary team, providing support to patients in accessing community resources and promoting independence.

The ideal candidate will have at least 2 years of community mental health experience and a relevant diploma or degree. You will facilitate early triage and assessments while ensuring continuity of care for patients facing mental health challenges.

Qualifications

• Minimum of 2 years community experience in mental health work.
• Experience in delivering brief interventions for mental health needs.
• A diploma or degree in appropriate subject.

Responsibilities

• Conduct assessments and support patients in identifying needs.
• Provide structured psycho-social interventions.
• Promote independence through an enabling approach.
• Coordinate access to community services.
