Qualifications

  • Must have a current professional registration.
  • Able to meet the travel requirements of the post.
  • Experience in Care Coordination.

Responsibilities

  • Complete assessments on patients referred within the locality.
  • Hold a caseload of patients and provide treatment.
  • Develop care plans with patients and the MDT.
  • Risk assess and manage patients as needed.
  • Participate in the duty system when required.

Skills

Knowledge of principles of assessment
Knowledge of risk assessment and management
Knowledge of Mental Health Act 1983
Experience in a mental health setting
Experience in managing own case load
Experience working within a multidisciplinary team
Experience in delivering evidence-based interventions

Education

Social work qualification
Nursing qualification
Occupational Therapy qualification
