East Sussex County Council is seeking a part-time Registered Mental Health Nurse to join the Joint Community Rehabilitation Service. The role involves assessing adults in the community, planning care, and maintaining independence for those with dementia or functional mental health needs.

You will work in a multidisciplinary team, triaging referrals, conducting assessments, and delivering nursing interventions to support safe, independent living at home.

Qualificações

• Registered with the Nursing and Midwifery Council.
• Strong experience assessing mental health and support needs of adults.
• Ability to work effectively within a multidisciplinary team.
• Knowledge of the Mental Capacity Act, safeguarding, and contemporary mental health practice.
• Clear written and verbal communication with a wide range of people.

Responsabilidades

• Triage referrals according to criteria and undertake initial assessments.
• Contribute to care planning and risk management for adults in the community.
