Job summary

Cheshire East Council is recruiting a Senior Support Worker to join the Care4CE Mental Health Reablement team in Macclesfield. This full-time, permanent role covers the north of Cheshire East with home visits and community referrals, Monday to Friday 9am–5pm, with occasional early evenings.

You will support adults with mental health or substance misuse issues to regain independence. Responsibilities include assessing risk, safeguarding, coordinating with local services, and maintaining accurate

Qualifications

• Experience in a health or social care setting and understanding of mental health issues.
• Ability to assess risk and apply safeguarding procedures.
• Knowledge of local service provision.
• Excellent communication, organisation, and record-keeping skills.
• Enthusiastic, flexible, reliable.
• Able to support the wider team and act as a point of contact for stakeholders.

Responsibilities

• Make appointments at the home address.
• Support referrals to engage in the community.
• Cover Macclesfield, Congleton and Wilmslow.
• Provide short-term practical support to adults.
• Assist with safeguarding and risk management.
