Job summary

Hamilton Woods Associates are recruiting a Referral Coordinator / Community Liaison Officer to join their established team in Birmingham. The role focuses on developing referral pathways, managing referrals from enquiry to placement, and building relationships with Local Authorities, NHS services and housing providers.

You will work with Housing and Support teams, monitor occupancy and ensure suitable placements, requiring experience in housing, homelessness or social care, and a full UK driving

Qualifications

• Experience within supported accommodation, housing, homelessness, social care, referrals or community engagement.
• Experience managing referrals, placements or assessments.
• Strong relationship-building and networking skills.
• Excellent communication and organisational skills.
• Experience working with Local Authorities, NHS services, Housing Providers or other professional agencies.
• Ability to work independently and manage multiple priorities.
• Full UK Driving Licence and access to a vehicle.
• Applicants must already have the legal right to work in the UK. Visa sponsorship is not available.

Responsibilities

• Develop referral pathways with Local Authorities, NHS services, Probation, Social Workers, Homelessness teams and charities.
• Build and maintain strong relationships with referral partners.
• Promote available accommodation and services to relevant professionals.
• Manage referrals from enquiry through assessment, viewing and placement.
• Attend stakeholder and networking meetings.
• Work closely with Housing and Support teams to ensure appropriate placements.
• Monitor referrals, occupancy and voids.
