Job summary

Tees Esk and Wear Valleys NHS Foundation Trust seeks a qualified Community Learning Disability Nurse / Health Facilitator (Band 5) to join the ALD Physical Health Team. The post is full-time, Monday to Friday, 9am–5pm and requires current NMC registration with preceptorship completed to be considered.

Qualifications

• Current NMC registration as RNMH or RNLD.
• Trained clinical supervisor or working towards within probation.
• Evidence of ongoing professional development.

Responsibilities

• Work with LD adults to assess health needs and enable access to services.
• Collaborate with health and social care colleagues in a multi-disciplinary team.
• Provide person-centred support and promote health equality.
