Tees Esk and Wear Valleys NHS Foundation Trust is seeking an experienced Band 4 Nursing Associate to join the DTV Adult Learning Disabilities Physical Health Team. The post supports CNS in implementing the STOMP agenda and delivering training to promote safe, person-centred care for people with learning disabilities.

You will be involved in medication reviews, health monitoring, and MDT collaboration to reduce health inequalities and improve outcomes. Right to work in the UK is required.

Competenze

• Registered Nursing Associate on the NMC Register.
• GCSE grades A-C in Maths and English or equivalent.

Mansioni

• Support CNS in delivering the STOMP agenda across primary and secondary care.
• Contribute to medication reviews and health monitoring for people with learning disabilities.
• Gather information and provide training to staff and carers.
• Deliver person-centred care and liaise with health and social care colleagues.
• Assist with MDT working and signpost to appropriate services.
