Job summary

Tees Esk and Wear Valleys NHS Foundation Trust is seeking a Band 4 Nursing Associate to join the DTV Adult Learning Disabilities Physical Health Team. The post involves supporting the CNS in delivering the STOMP agenda across primary and secondary care, contributing to medication reviews and health monitoring.

You will work under the CNS, provide training and signposting, promote person-centred care, and participate in MDT work to reduce health inequalities for people with learning disabilities.

Qualifications

• Registered Nursing Associate on the NMC Register is essential.
• GCSE Maths and English or equivalent is required.
• Experience in learning disability or mental health settings is desirable.

Responsibilities
