South West Yorkshire Partnership NHS Trust is seeking a Community Learning Disability Nurse (Band 5) for Wakefield Adult Community LD Health Service. The post supports adults with learning disabilities, in a multi-disciplinary team at Fieldhead Hospital, with emphasis on reducing health inequalities and promoting optimum health.

The role requires current nursing registration (LDN or RMN with LD experience), a proactive approach, and ability to work with complex needs.

Qualifications

• Registered Learning Disability Nurse or RMN with LD experience.
• Experience mentoring learners or trainees.
• Flexible working hours and commitment to safeguarding.

Responsabilités

• Assess and plan care for adults with learning disabilities on a defined caseload.
• Work within a multidisciplinary team to provide skilled interventions.
• Maintain risk assessment and care planning in line with policies.
• Provide professional nursing care and promote health improvement for service users.
