Job summary

Tees Esk and Wear Valleys NHS Foundation Trust seeks a Qualified Community Learning Disability Nurse / Health Facilitator (Band 5) to join the LD health team. The post focuses on enabling access to health services, promoting the health needs of adults with learning disabilities, and reducing health inequalities.

The role is full time, with standard hours Monday to Friday 9am–5pm, and involves collaboration with wider health colleagues, local authorities and primary/secondary care to deliver

Qualifications

• Current professional registration with NMC as RNMH or RNLD.
• Evidence of Continuing Professional Development (CPD).
• Ability to work in a person-centred therapeutic team.
• Strong collaboration with multidisciplinary teams and carers.

Responsibilities

• Promote health needs of adults with learning disabilities and enable access to wider health services.
• Work with LD Community Teams and Local Authority to reduce health inequalities.
• Provide guidance within a band 5 Community Nurse/Health Facilitation role.
• Support carers and patients through partnerships with primary and secondary care.
