Zusammenfassung

Torbay and South Devon NHS Foundation Trust is seeking an experienced community nurse to join the intermediate care team in Torquay. You will care for housebound patients recently discharged from hospital or whose health has declined, working with GPs and social care to restore function and independence.

Applicants must be a registered adult nurse with NMC. Prescribing and palliative care training are desirable, and you will work within a multidisciplinary team to deliver high-quality,

Qualifikationen

• Level 1 adult nursing qualification.
• NMC registration as an adult nurse.
• Proven experience across a range of clinical skills.
• Experience communicating complex information to patients, carers and health and social care staff.
• Relevant team working experience.

Aufgaben

• Support housebound patients recently discharged from hospital or with a decline in medical status within the intermediate care team.
• Deliver high quality individualised care to housebound patients.
• Work in a multidisciplinary team and in partnership with primary and secondary care.
• Promote patient comfort, well-being and rehabilitation.
• Communicate with GPs, hospital, community, out-of-hours, third sector services and care homes.
• Generate prescriptions and order pressure relieving equipment using IT systems.
• Meet other objectives agreed at the Achievement Review.
