Job summary

Cornwall Partnership NHS Foundation Trust is seeking a compassionate Community Nurse (Band 5) to join the Launceston and Tamar Valley team. You’ll provide home-based, patient-centred care across Callington and Launceston with a multidisciplinary team, delivering high-quality nursing care in patients’ own homes.

You will assess and plan care, promote independence and work with GPs, therapists and social care colleagues to support wellbeing. Flexibility for travel and varied shifts is required.

Qualifications

• Registered Nurse (Adult) with current NMC registration.
• Evidence of recent CPD and ongoing professional development.
• Mentor qualification or willingness to work towards it.

Responsibilities

• Assess, plan, implement and evaluate care in patients' homes and communities.
• Perform holistic assessments and develop personalised care plans.
• Collaborate with GPs, therapists and social care teams to provide seamless care.
