Job summary

Cornwall Partnership NHS Foundation Trust is seeking a Community Nurse for the Launceston and Tamar Valley Community Nursing Team. The role involves delivering high-quality, patient-centred care in patients' homes across Callington and Launceston, working within a supportive multidisciplinary team.

You will assess, plan, implement and evaluate care with GPs, therapists, and social care colleagues, promoting independence and wellbeing in the community.

Qualifications

• Registered Nurse with current NMC registration.
• Experience in community or district nursing is desirable.
• Excellent communication and patient-centred care skills.

Responsibilities

• Assess, plan and provide care for patients in their homes across Callington and Launceston.
• Collaborate with GPs, therapists, social care staff and other partners to support independence and wellbeing.
• Travel between patient homes, clinics and community settings as required.
• Deliver evidence-based nursing care and holistic assessments within community teams.
