Job summary

Lancashire & South Cumbria NHS Foundation Trust is seeking a Registered Nurse (General/Adult) to deliver high-quality care in patients’ homes. You will work within a multidisciplinary team, perform holistic assessments, wound care, and palliative/end-of-life care, and support discharge planning under supervision.

Flexibility is valued, with possibilities for varied working patterns and rotation across a seven-day service. Experience in community health and health education is advantageous.

Qualifications

• Post-registration experience working in a multidisciplinary team.
• Experience in holistic assessment of needs and care planning.
• Experience delivering wound care, palliative and end-of-life care.

Responsibilities

• Undertake delegated caseload under supervision; assess needs and discharge patients as appropriate.
• Coordinate holistic assessment and planning of care packages.
• Provide high-quality palliative care to patients and families.
• Collaborate with other health and social care professionals to meet patient needs.
• Prescribe from the relevant Prescribing Formulary if non-medical prescriber.
• Conduct clinical risk assessments.
• Contribute to public health initiatives and health needs assessments.
• Promote health education and healthy lifestyles within the community.
