Job summary

Cambridgeshire and Peterborough NHS Foundation Trust (CPFT) is seeking a Band 5 Community Nurse to deliver high-quality care in patients' homes and residential settings across Cambridge East and South City areas. You will be supported to develop wound care, palliative care, and long-term condition management, while delivering compassionate care.

The role requires a full UK driving licence and access to a vehicle, with a 37.5-hour week and weekend work.

Qualifications

• Experience in community nursing or care in home settings.
• Educated to RGN level and registered nurse.
• Ability to teach and support student nurses and care workers.

Responsibilities

• Place patient at the centre of care and involve patient and carer in decisions.
• Provide high standard nursing care in patient homes with access to specialists as needed.
• Promote independence and reduce unnecessary hospital admission where possible.
