Job summary

This role requires strong clinical skills, autonomous decision‑making and the ability to travel across a large geographical area. A driving licence and access to a vehicle are essential.

Qualifications

• Registered nurse with current NMC registration.
• Experience in community nursing or home visits preferred.
• Willing to travel across a large geographical area.
• Excellent clinical and communication skills.
• Able to work independently with autonomous decision-making.

Responsibilities

• Prioritise care based on clinical need and health status.
• Undertake holistic assessments and develop care plans.
• Deliver palliative and end-of-life care where needed.
• Manage catheters and related procedures.
• Assess and treat wounds (acute and chronic).
• Support long-term condition management.
• Collaborate with multidisciplinary teams and partner agencies.
• Mentor and supervise colleagues, including workload allocation.
• Contribute to service development and improvements.
• Work across a large geographic area with a valid driving licence.
