Job summary

Sussex Community NHS Foundation Trust's Adur Community Primary Care Network Team in Lancing is recruiting a Band 5 Community Nurse to provide high-quality care to housebound patients in their homes and care homes.

You will perform comprehensive nursing duties including wound care, catheter care, medication administration, and support patients with long-term conditions and end-of-life care. Structured induction and ongoing support will help you develop within a friendly team.

Qualifications

• Registered Nurse (Band 5) in community setting.
• Experience in community nursing and care for housebound patients is desirable.
• Wound care, long-term condition management, palliative care, catheter care, safeguarding, and care home support.

Responsibilities

• Assessment, planning, delivery and evaluation of patient care.
• Wound assessment and management.
• Catheter and continence care.
• Administration of medication, including insulin where competent.
• Supporting patients with long-term conditions and complex health needs.
• Delivering palliative and end-of-life care.
• Safeguarding adults and raising concerns where appropriate.
• Working with GPs, therapists, care homes and the multidisciplinary team.
• Supporting hospital discharge and preventing unnecessary hospital admissions.
• Maintaining accurate clinical records and contributing to care planning.
