Job summary

Essex Partnership University NHS Foundation Trust is seeking a Community Support Worker for Older Adults to join the Mid Dementia and Frailty Pathway. The role requires a Band 3 level and a care certificate, and you will support the multidisciplinary team in promoting physical and mental wellbeing.

You will drive between Mid Essex sites, complete health checks, and contribute to developing care plans. Full-time, 9-5, Monday to Friday, with opportunities for flexible working and staff support.

Qualifications

• Essential criteria include a valid UK driving licence and access to a car.
• Band 3 role with care certificate requirement.
• IT literacy to support admin tasks and MDT work.

Responsibilities

• Support multidisciplinary team in dementia and frailty pathway.
• Undertake annual physical health checks and document findings.
• Develop and update care plans with service users and carers.
• Promote health and wellbeing, including health promotion activities.
