Southern Health NHS Foundation Trust in Winchester is seeking a Healthcare Support Worker to join the Older Persons Mental Health Community Team. You will provide diagnostic support for dementia and help individuals with other mental health diagnoses stay connected to their community and maintain independence at home.

You will support families and carers, work as part of a compassionate team, demonstrate motivation and a genuine interest in caring for older people, and have a full UK driving

Formación

• Motivated and compassionate team player with an interest in working with older people.
• Ability to deliver high quality care and support to patients and families.
• Full UK driving licence and access to a car.

Responsabilidades

• Provide diagnostic support for patients with dementia.
• Support individuals with other mental health diagnoses to stay connected to the community and maintain independence at home.
• Signpost families and carers to appropriate services and support networks.
• Work as part of the Older Persons Mental Health Community Team in Winchester.
