Job summary

Tees Esk and Wear Valleys NHS Foundation Trust is seeking a Community Mental Health Support Worker to assist older adults in the community. You will work alongside a multidisciplinary team, delivering therapeutic and practical support and building knowledge of local services to help patients and families.

You will travel across a large geographic area, communicating flexibly with diverse service users and their carers, under supervision with opportunities for professional development.

Qualifications

• Diploma in Health and Social Care Level 2 or equivalent.
• Care Certificate.
• Literacy and Numeracy Level 2.
• Complete the Trust's full HCA Framework within 6 months.

Responsibilities

• Provide ongoing support and practical assistance in the community.
• Deliver non-pharmacological interventions with individuals and groups.
• Monitor mental and physical health under supervision of registered staff.
• Support patients and carers to identify and review health needs and goals.
