Job summary

The YOU Trust is seeking a Community Health Worker to support people across Weymouth, Portland and the West of Dorset on a 9-month, 25-hour-per-week contract. A driving licence is required to travel to client homes and community venues.

You will manage a caseload, assist with housing and benefits applications, budgeting, and help clients access local services. Expect travel across Dorset and strong commitment to welfare and independence.

Qualifications

• Understand homelessness risk factors and impact on wellbeing.
• Knowledge of housing-related support, housing law, debt, welfare benefits, welfare reform and universal credit.
• Experience with a diverse range of client groups.

Responsibilities

• Manage own caseload, providing one-to-one support in clients' homes or community.
• Support clients via drop-ins local to work area.
• Assist with housing applications, benefits and budgeting.
• Encourage access to resources supporting health and wellbeing (community services, groups, peer support).
• Partner with agencies and advocate on behalf of clients.
• Attend regular local office meetings in Dorchester.
• Travel across Dorset to meet clients' needs.
• Build positive relationships with community and support networks.
