Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking a District Nurse to provide high-quality community nursing at patients' homes, care homes and supported living environments. The role includes medication administration, diabetes and wound care, end-of-life care, and promoting self-management.

The nurse will work across three weekdays with a weekend rota in a seven-day service and will travel widely by vehicle.

Qualifications

• Qualified or soon-to-be-qualified adult nurse.
• Evidence of recent relevant training.
• NMC registration, or PIN awaited.

Responsibilities

• Provide high-quality community nursing to keep patients safe at home and support discharge.
• Assess, plan, implement and evaluate individualised patient care.
• Administer medication and manage diabetes.
• Provide wound care and leg ulcer management.
• Provide end of life care and catheter care.
• Offer self-management education and promote independence.
• Work autonomously within the community nursing team and with GPs and hospitals.
• Adhere to NMC code of conduct and trust policy.
