Derbyshire Community Health Services NHS Foundation Trust is seeking two Part-time Community Nurse posts based in Chesterfield and Dronfield Integrated Community Team. Roles cover home visits, clinics and care homes, with 30h or 22.5h per week on a 7-day service rota.

Post holders will visit patients in their homes, manage diabetes, wounds, leg ulcers, catheters and end-of-life care, and promote self-management. Travel and a UK driving licence are essential.

Competenze

• This role requires valid NMC registration or pending PIN.
• Qualified or soon to be qualified adult nurse with NHS community experience preferred.
• Recent training in nursing practice and safeguarding.

• Provide high quality nursing care in patients' homes and communities.
• Assess, plan, implement and evaluate individualised care.
• Adhere to NMC code and trust policies.
• Maintain privacy and dignity while working in multi-disciplinary teams.
• Support junior staff and liaise with GPs and hospitals.
