Procare Community Services seeks a District Nurse to manage a caseload through home visits and care home visits, providing comprehensive nursing care and wound management. The role focuses on long-term conditions, palliative care, and interdisciplinary collaboration.

Applicants must hold a District Nursing qualification, current NMC registration, and have a driving licence with access to a vehicle for business use.

Qualificações

• District Nursing specialist practice qualification (diploma or degree).
• Assessor or mentorship qualification.
• Current NMC registration as a Level 1 nurse.

Responsabilidades

• Visit patients in their homes and care homes to manage a defined caseload.
• Provide advanced clinical care including wound care and catheterisation.
• Collaborate with GPs, nurses and social services in the community.
• Lead and mentor staff and students within the community team.
• Attend MDT meetings and engage in health promotion activities.
