Job summary

Salford Royal NHS Foundation Trust is seeking a motivated Community Staff Nurse to join the Salford Community District Nursing Team based at Eccles District Nurses. You will manage your own caseload, work with a multidisciplinary team, and deliver holistic care in patients' homes.

A six-week induction and robust mentoring support are provided to ease transition into community settings. Applicants must have post-registration experience, completed their nursing preceptorship, and hold a current

• Must have proven post-registration nursing experience.
• NMC PIN to practise in the UK is required.

Responsibilities

• Manage own caseload in patients' homes within the community.
• Work closely with the MDT (GPs, Social Workers, Allied Health Professionals) to coordinate care.
• Provide holistic, person-centred nursing in community and clinic settings.
