• Provides community-based, individualized care including long-term condition management.
• Performs venepuncture and medication administration as part of care standards.
• Completes nursing assessments and follows prescribed treatment regimens.

Responsibilities

• Work in a multi-professional team to deliver patient-centered care.
• Manage own workload with guidance from line manager and delegate tasks to junior colleagues as appropriate.
• Able to travel across the locality and work in patients' homes as required.
