Responsibilities

• Provide district nursing care to housebound patients in the East Primary Care Network.
• Run a District Nurse clinic for non-housebound patients with specialist District Nurse skills.
• Support patients towards self-management through education, equipment and lifestyle changes.
• Support patients and families at the end of life, liaising with hospice teams and palliative care nurses.
• Work with GP colleagues, multi-disciplinary teams, care homes and care agencies.
• Record all work electronically on EMIS connected to GP services.
