Qualifications

• Registered nurse with district nursing experience and current UK registration.
• Experience leading and managing a community nursing team.
• Mentor/preceptor qualification or willingness to train.
• Nurse prescribing qualification or willingness to train.
• Commitment to professional development.

Responsibilities

• Provide support to a multidisciplinary team delivering clinical health and social care in admission prevention services.
• Set and maintain standards of care; deploy, co-ordinate and supervise nursing team and learners.
• Assess, plan, implement and evaluate individualised nursing care in the community.
• Maintain accurate patient records.
• Participate in long-term management of patients within the locality.
• Manage a defined caseload.
