Job summary

Hertfordshire Community NHS Trust is seeking a Band 6 Caseload Manager to lead a multidisciplinary community nursing team in Royston. The post requires autonomous leadership, strong communication, and a commitment to high-value patient care within a seven-day service.

The successful candidate will manage a caseload, supervise Band 5–7 nurses, and contribute to ongoing professional development and service improvements. A full UK driving licence is essential.

Qualifications

• Registered Nurse with current UK registration.
• Experience at Band 5 in community nursing.
• Experience supervising a team and managing change.
• Ability to mentor others and participate in teaching.

Responsibilities

• Provide support to a team of multidisciplinary professionals in community health and social care.
• Set and maintain standards of care and deploy, coordinate and supervise the nursing team and learners.
• Assess, plan, implement and evaluate individualized nursing care in the community.
• Maintain accurate patient records and contribute to long-term case management.
• Provide ongoing responsibility for a defined caseload in the locality.
