Job summary

Hertfordshire Community NHS Trust is seeking a Band 6 Caseload Manager to lead a Community Integrated Team in Royston. The role focuses on high-value, community-based nursing and multidisciplinary collaboration within an 8am–10pm seven-day service.

Applicants should be an experienced registered nurse with leadership abilities, able to supervise a mixed band team and maintain clinical governance standards while supporting professional development.

Qualifications

Must be a Registered Nurse with community nursing experience.

Responsibilities

Provide support to a team of multidisciplinary professionals in admission prevention/avoidance service.
Set and maintain standards of care; deploy, co-ordinate and supervise nursing team and learners.
Carry out assessment, planning, implementation and evaluation of individualized nursing care in the community.
Maintain accurate patient records and contribute to long-term management of patients in the locality.
Manage a defined caseload and demonstrate capability to lead changes in a complex environment.
Demonstrate flexibility and autonomous working with strong communication skills.
