Job summary

Shaw Trust in Norfolk invites applications for a Work and Health Coach role. You will deliver early intervention and health support to individuals, helping them overcome barriers to employment and linking to other services.

You'll manage a caseload with tailored, one-to-one support in a community setting. You will collaborate with MDTs, GP practices and referral partners, supporting clients to return to work or find new employment while improving health and wellbeing.

Qualifications

• A genuine passion for helping others achieve their potential.
• Excellent communication and interpersonal skills.
• Experience working with people with health needs.
• Knowledge of employment services or recruitment is a plus.

Responsibilities

• Provide early intervention work and health support to individuals.
• Manage a caseload with tailored, one-to-one support.
• Work with the MDT to discuss cases and refer to partners.
• Collaborate with health teams, GP practices and referral partners to support return to work.
• Help participants remove health barriers by sourcing interventions.
