Job summary

The Growth Company in the United Kingdom is recruiting to support participants facing health barriers toward sustained employment. You will manage a caseload, conduct Work and Health Assessments, and develop personalised action plans with employers and partners to enable progress toward employment goals.

We seek empathetic team players who can work independently and build trust with participants, healthcare partners, and local employers.

Qualifications

• Excellent interpersonal and communication skills used to support individuals facing barriers to employment.
• Ability to manage a caseload and prioritise competing demands effectively.
• Experience delivering guidance, advice, coaching or person-centred support.
• Knowledge of challenges faced by individuals with health conditions or other barriers to employment.
• Ability to build professional relationships with partners and stakeholders.
• Understanding of local support services and referral pathways.
• Accurate administration and data management in line with requirements.
• Proficiency with MS Office and digital case management tools.

Responsibilities

• Manage a caseload of participants and provide tailored support to achieve employment goals.
• Conduct Work and Health Assessments in person or online to identify needs.
• Develop individual action plans to help participants overcome barriers.
• Source job opportunities through employer engagement and local labour market knowledge.
• Collaborate with NHS services, GP practices and community partners to support outcomes.
• Deliver triage and refer participants to appropriate services.
• Support employers with workplace adjustments and return-to-work plans.
• Maintain accurate records and case notes per programme policies.
