Job summary

Sheffield City Council is seeking an Adult Social Care Practitioner for the Communities North Team within the Living and Ageing Well service. You will assess and plan care for adults 65+, using a strengths-based, person-centred approach and working with families, carers and health partners.

The role involves home visits, risk assessment, and producing clear assessments and support plans in line with care legislation. DBS checks and ongoing professional development are required.

Qualifications

• Experience applying the Care Act 2014 in adult social care.
• Demonstrated ability to assess care and support needs and manage risk.
• Ability to produce clear, evidence-based assessments and support plans.
• Knowledge of safeguarding and Mental Capacity Act principles.

Responsibilities

• Undertaking strengths-based Care Act assessments and identifying eligible needs.
• Developing care and support plans with people and carers.
• Carrying out reviews to ensure ongoing suitability of support.
• Identifying and responding to risk and safeguarding concerns.
• Applying Mental Capacity Act principles and contributing to best-interest decisions.
• Undertaking home visits across Sheffield and maintaining accurate records.
