Job summary

Belmont Recruitment are seeking an experienced Adult Social Care Practitioner to join Sheffield City Council's Reablement Team on an initial 6‑month temporary contract. This full‑time role is 37 hours per week, Monday to Friday, based in Sheffield.

You will assess the needs of adults requiring social care support, develop person‑centred care and support plans, and complete reviews to promote independence, wellbeing and positive outcomes.

Qualifications

• NVQ Level 3 or equivalent in Health and Social Care.
• Experience delivering care and support to adults with disabilities or health conditions.
• Knowledge of safeguarding procedures and relevant legislation (Care Act 2014, Mental Capacity Act 2005).
• Experience completing assessments, care planning and reviews.

Responsibilities

• Complete Care Act 2014 assessments to identify eligible care and support needs.
• Develop person-centred care and support plans with individuals and carers.
• Conduct reviews to ensure needs are met and outcomes achieved.
• Carry out risk assessments and implement appropriate safety measures.
• Promote independence, wellbeing and choice through a strengths-based approach.
• Respond to safeguarding concerns in line with policies and procedures.
• Provide information and signposting to housing, welfare benefits and other support services.
• Undertake home visits across Sheffield to assess needs and provide support.

You will work collaboratively with individuals, carers, health professionals and community organisations, ensuring support is delivered in line with the Care Act 2014.

Key Responsibilities

• Complete Care Act 2014 assessments to identify eligible care and support needs.
• Develop person-centred care and support plans in partnership with individuals and their carers.
• Conduct reviews of care and support arrangements, ensuring needs are being met effectively.
• Carry out risk assessments and identify appropriate measures to reduce risks.
• Promote independence, choice, recovery and wellbeing through a strengths-based approach.
• Identify and respond to safeguarding concerns, following relevant policies and procedures.
• Provide information, advice and signposting to housing, welfare benefits and other relevant support services.
• Undertake home visits across Sheffield to assess needs and provide support.

Requirements

• NVQ Level 3 or equivalent relevant qualification, or the ability to demonstrate competence at this level.
• Experience working with adults with physical disabilities, mental health conditions or other social care needs.
• Knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures.
• Experience completing assessments, care planning and risk assessments.
