Opus People Solutions Limited is recruiting a Senior Social Work Practitioner for the North Community Team. Based in North Shropshire (Market Drayton & Oswestry), the role supports adults with disabilities, older people, and their carers, taking a lead on assessments and care planning.

This hybrid role requires a strong commitment to person-centred care, safeguarding, and interagency collaboration, with a 37-hour week and pay of £31.97 per hour umbrella.

Qualifications

• Experience conducting comprehensive assessments under Care Act 2014.
• Ability to manage a caseload of adults with disabilities and older people.
• Experience with safeguarding investigations and multi-agency procedures.
• Deliver person-centred care and promote independence.

Responsibilities

• Undertake comprehensive assessments and fulfil statutory responsibilities under the Care Act 2014.
• Manage and support a caseload of adults with disabilities, older people, and their carers.
• Contribute to Multi-Agency Protection of Vulnerable Adults procedures and safeguarding investigations.
• Deliver high-quality social work interventions within relevant legislative frameworks, council policies, and partnership procedures.
• Apply a strengths-based approach to assessments, care planning, and reviews.
• Support individuals through hospital discharge processes, ensuring safe, timely, and effective transitions back into the community.
• Work collaboratively with health professionals, partner agencies, service users, and carers to achieve positive outcomes.
• Promote customer engagement and participation, ensuring individuals have a voice in decisions affecting their lives.
• Maintain accurate records and professional standards in line with statutory requirements and best practice.

This role is exempt from the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975. As this role is eligible for an Enhanced DBS check, applicants are required to declare any cautions and convictions that are not protected under current filtering rules.
