Belmont Recruitment are seeking an experienced Adult Social Care Practitioner to join Sheffield City Council's Reablement Team on a 6-month temporary contract. The role is full-time, 37 hours per week, Monday to Friday, based in Sheffield.

You will complete Care Act assessments, develop person-centred care plans with individuals and carers, and conduct reviews to ensure needs are met. The post involves home visits and safeguarding responsibilities in a supportive, outcomes-focused setting.

Formación

• NVQ Level 3 or equivalent qualification.
• Experience working with adults with physical disabilities, mental health conditions or other social care needs.
• Knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures.
• Experience completing assessments, care planning and risk assessments.
