Job summary

Wokingham Borough Council is seeking a Social Care Practitioner to join the Adult Social Care team. This permanent, part-time role (37 hours per week) supports adults to live safely and well in their communities, with supervision from a social worker/occupational therapist.

The role offers a clear progression to Grade 6 and a strong benefits package. You'll work to understand individual needs, carry out strengths-based assessments, and build positive relationships with residents and carers.

Qualifications

• Experience in health or social care, or in a people-facing role.
• Strong value base; respectful and trustworthy.
• Good IT skills and ability to use adult social care recording systems.

Responsibilities

• Working with people to find out what is important to them.
• Carrying out strengths-based assessments and reviews of daily lives and support arrangements.
• Gathering information from individuals, carers, professionals and advocates as needed.
• Writing reports and keeping accurate records.
• Building positive relationships with residents and supporting various communication needs.
• Working closely with colleagues as part of a team.
